University of South Alabama and USA Health employees who want to make a change in their health insurance plan or re-enroll in flexible spending accounts have until the Monday after Thanksgiving to submit the forms to human resources.

Open enrollment began Nov. 1 and ends at 4:30 p.m. on Monday, Nov. 30.

Here are several items to consider:

  • During open enrollment, eligible employees may enroll in a health insurance plan and add eligible dependents.
  • The USA Select Health & Dental Plan, administered by Blue Cross Blue Shield of Alabama, is replacing the USA VIVA Health & Dental Plan as a low-cost alternate plan. Employees currently enrolled in the VIVA plan will be automatically enrolled in the new plan.
  • For employees currently enrolled in the USA Health & Dental Plan who do not wish to make a change in coverage, no action is required.
  • Employees must re-enroll each year to continue to participate in SouthFlex, the medical and dependent care flexible spending accounts plan, administered by HealthEquity.
  • The USA Health & Dental Plan will have an increase in employer and employee cost sharing for 2021 following a year of continued escalating healthcare costs.

You may download forms on the Open Enrollment section of the human resources website. You can then pick up forms from human resources offices or call to request a form. Completed forms should be scanned and emailed to employeebenefitshr@southalabama.edu; or alternatively, forms may be hand-delivered to human resources. Faxed forms will not be accepted.

For more information, refer to the USA Employee Benefits & Wellness Open Enrollment guide, the Open Enrollment website or contact human resources at 251-460-6133.
