Medical Records

To request a copy of your records, please call the location(s) where you received care and ask for the Records Department. Patients who have registered for our Patient Portals also can log in to download copies of their records.

Patient Feedback

If you have a concern about patient care or safety in one of our hospitals, please ask for the Departmental Manager or House Supervisor. Every effort will be made to address your concern. If you are not satisfied with the response you receive, if you had a negative experience at one of our clinics, or if you want to share a positive experience, you can reach our Patient Relations teams using the following contact information or submit a message using our form below. Teams are available weekdays from 8 a.m. to 4 p.m. and will return your message as soon as possible.
