To request a copy of your records, please call the location(s) where you received care and ask for the Records Department. Patients who have registered for our Patient Portals also can log in to download copies of their records.

If you have a concern about patient care or safety in one of our hospitals, please ask for the Departmental Manager or House Supervisor. Every effort will be made to address your concern.
