USA Health employees are required to receive an annual/seasonal influenza vaccine or provide proof of an approved medical or religious exemption as part of their condition of employment.

As part of your condition of employment with USA Health, you are required to receive an annual/seasonal influenza vaccination or provide proof of an approved medical or religious exemption.  

This year USA Health is offering the following methods to help you fulfill this requirement of employment: 

Influenza Vaccine Clinics will be held at the following locations, dates and times:

    • Children’s & Women’s Hospital, Panda Cares Room, Oct. 14 and Oct. 16, from 7 a.m. to noon
    • University Hospital, second-floor Café Conference Room, Oct. 21, 22 and 23, from 7 a.m. to noon
    • Providence Hospital, DePaul Center, Oct. 7, 8 and 9, from 7 a.m. to noon
    • Mitchell Cancer Institute, Conference Room B, Oct. 10, from 7:30 a.m. to 12:30 p.m.
    • Strada Patient Care Center, Room 1101, Oct. 2, from 7:30 a.m. to 12:30 p.m.
    • Research Park III, IT Conference Room 1803, Oct. 17, 8:30 to 11:30 a.m.
    • Mobile Diagnostic Center/Physician Office Building, second-floor conference room, Oct. 30, from 7:30 a.m. to 12:30 p.m.
    • USA Health Office Park (UHOP), Conference Room 501, Oct. 28, from 8 a.m. to 12:30 p.m.

    Influenza Vaccine Drive-Thru Clinics will be held Sept. 15 through Oct. 31. Flu vaccines will be available for any employee via drive-thru service at 1720 Center St. (building in the front parking lot of Children’s & Women’s Hospital). The drive-thru clinic will be open Monday through Friday from 7 a.m. to 3 p.m. and does not require an appointment.  

    Should you choose to receive your vaccine outside of the previous options, you may upload proof of vaccination directly to your Employee Health Portal by clicking the following link: https://ul.pureohs.com. If required, provide USA Health Organization ID #4405. 

    Medical or religious exemptions are approved on a case-by-case basis. Medical exemptions require a physician’s signature. If you wish to submit an exemption for approval, please upload the required documentation to your Employee Health Portal by clicking the following link: https://ul.pureohs.com. If required, provide USA Health Organization ID #4405. 

    To streamline vaccine administration and documentation of compliance, we ask that you receive your vaccine utilizing one of the above methods. Employee Health will not be distributing vaccines outside of the sites and our drive-thru service.

    The deadline to receive the vaccine, provide proof of vaccination, or provide proof of exemption is Friday, Oct. 31.
