For general questions about our facilities, please call the numbers below. To look up contact information for outpatient clinics that are part of the USA Health Physician Enterprise, please visit our Locations Directory. Patients who have registered for our online Patient Portals also can log in to message their providers directly. 

 

Medical Records

To request a copy of your records, please call the location(s) where you received care and ask for the Records Department. Registered users of our Patient Portals also can log in to download copies of their records. 
 
 

Billing Questions

Billing for USA Health's hospitals, outpatient locations, the Mitchell Cancer Institute, and anesthesia is handled by several different departments. Please visit our Bill Pay page to find the correct phone number and online payment link for your bill.

 

Patient Feedback

If you have a concern about patient care or safety in one of our hospitals, please ask for the Departmental Manager or House Supervisor. Every effort will be made to address your concern. If you are not satisfied with the response you receive, if you had a negative experience at one of our clinics, or if you want to share a positive experience, you can reach our Patient Relations teams using the following contact information. Teams are available weekdays from 8 a.m. to 4 p.m. and will return your message as soon as possible.
