Employee testing: Please refer to the Employee Testing Guide [PDF] to determine whether testing is required.  Employees can request a testing appointment using the COVID-19 Testing Form. Once the form is submitted, you will be able to self-schedule your appointment. Depending on your results, you may need to complete the Return to Work Attestation for Negative Employees [PDF] or refer to the Return to Work Instructions for Positive Employees [PDF].

Vaccination appointments: USA Health employees (or their managers) can self-schedule vaccination appointments at usahealthvaccines.com. If you are having technical difficulties scheduling your appointment or with your vaccination card please submit your information here.

    Inpatient Testing Policy

    All patients admitted to UH and CW will have COVID-19 testing upon admission. COVID-19 testing power plans have been developed to guide providers in selecting the appropriate isolation orders and COVID-19 lab orders. Three different power plans have been developed -- one each for University Hospital, PEDs, and OB/GYN -- and each has been embedded into the appropriate admission order sets. This document has been created to further explain these order sets [PDF].
