We are conducting a research study about how to support young adults with sickle cell disease as they prepare for moving to adult care. We will compare a structured education-based program with a peer mentor to a structured education program alone to see if it reduces how often young adults with sickle cell disease go to urgent care, the emergency department, or get admitted to the hospital.
To participate you will be asked to:
- Complete questionnaires about your quality of life every 6 months
- See your sickle cell provider every three months for at least 30 months
- Your providers will track how often you go to clinic appointments, urgent care, the emergency department, and get admitted to the hospital
- You may be assigned a peer mentor who will contact you regularly to support you during the transition process.
