Copies of Medical Records:

To obtain copies of your medical records, please call us at 415-2246, Monday-Friday, between 8 a.m. and 4:30 p.m. and an employee will explain the steps to follow for obtaining copies of medical records.

You will need to complete the Patient Authorization for Use and/or Disclosure of Protected Health Information form.  You can download the form here:

Also, to adequately verify your information, we will need you to present a valid photo ID at the time of the request.

Birth Certificates:

For questions regarding the completion status of a birth certificate, please call 415-1632. 

To Obtain Copies of Birth Certificates:
Alabama Birth Certificates can only be issued by the Mobile County Health Department, 690-8151, and by the Alabama Department of Public Health in Montgomery, (334) 206-5418.

Social Security Cards & Birth Announcements:

Completion Status:
For questions regarding the completion status of your child's Social Security card, please contact the Social Security office at 1-800-772-1213.  For questions regarding your child's birth notification in the paper, please contact the Mobile Press Register at 219-5709. 

Death Certificates:                 

Completion Status:
For questions regarding the completion status of a death certificate, please call 415-1632.  If the Mobile County Coroner has declared a death a Coroner's Case, the death certificate will be completed by the coroner's office and not by USA Children's & Women's physicians.  Please call 471-3186 to contact the Mobile County Coroner's office about these death certificates.
